This actually happened some time ago.
I was trying to trim up my displacement piston on my Stirling engine. I needed to take a bit off the end as it was coming in contact with my cylinder at high rpm. I'd already removed a hundredth or so off the end. I went to make one more light pass and the cutting tool ripped it out of the chuck.
I got to spend the rest of the day making a new one.
