A drill chuck comes off of it's taper at the end of a drill cycle, remaining in the hole, and the next tool in the cycle slams into the chuck while your back is turned.

The parts were nothing special, a many-piece run in G10 fiberglass. The drill and chuck were unhurt, and the only casualty was the next tool, a small carbide centerdrill.
Not something you see every day. :big:
 
And yet another reason to not turn away from a machine while it is in motion. ;D

I saw seen a very similar incident at work one day.
It happened in the machine our Sr. CNC operator was running.
It sounded like a freight train wreck, but this guy is GOOD!
He was on the cycle stop button at the first indication of
something going wrong. When the coolant stopped spraying
we saw a drill bit with the collet holder still attached sticking
out of the stock in the chuck and a boring bar just barely making
contact with the collet holder.

His response was....

"That ain't right."
